Improving Health & Building Community through Centering Groups: HCSC Grant Opportunity

Imagine a two-hour prenatal care or well-child visit where participants receive the highest quality clinical care, benefit from peer support, facilitated discussion and experiential learning, and are empowered as they learn from their healthcare team and one another?

During this session, we will provide a high-level interactive overview of Centering, an evidence-based, patient-centered model of group care that has been proven to have a positive impact on health outcomes and racial health disparities. The session will emphasize CenteringPregnancy®️, a nationally recognized model of group prenatal care shown to reduce the risk of preterm birth, and CenteringParenting®️, a two generation, group well-child intervention that supports healthy parent-child interactions, behavior modeling, and early learning. Participants will gain an understanding of how the Centering care model combines health assessment, interactive learning, and community building to help support positive health behaviors and drive better health outcomes. In addition, participants will learn how Centering group visits reduce social isolation and create a community of support, empowering patients, parents & families with knowledge and skills to support optimal health & development.
